Requirements

• Class A CDL, age 21 or older, with at least 3 months of verifiable driving experience

• Live within 50 miles of Blue Ash, OH or Monroe, OH

• No more than 1 preventable CMV accident in the last 2 years, and no major preventable CMV accidents in the last 5 years

• No more than 2 moving violations in the last 2 years

• No DUI/DWI in the past 5 years (10 years if it happened while holding a CDL)

• Must pass a DOT physical and drug screen; no drug-related conviction in the past 5 years (10 years if while holding a CDL), and no positive or refused drug tests in the past 10 years as a CDL-A holder

• Drivers in the SAP program are not eligible
